I worked for a major communication company and saw just that way too many times.

That cable right there is a lead sheath cable that is wrapped in tar and burlap, it looks just like a root when it is exposed. Those conductors are insulated in paper, no color code to put them back together. You had to send a tone from both ends to mate the correct ones up with each other.
